Tax Forms & Government Documentation

T4 · T5 · RC66

Government forms can be confusing and a small mistake can cost you. I fill them out properly, from year-end slips to benefit applications and corrections to past returns.

T4, T4A, T5 slips, RC66 child benefits application, CRA adjustments and more.

Who this is for

You need a specific form prepared and filed correctly — as an employer, as a corporation paying dividends, or as an individual claiming a benefit you are entitled to.

What's included

  • T4 and T4 Summary — employment income
  • T5 and T5 Summary — investment income and dividends
  • RC66 Canada Child Benefits application, with the RC66SCH where required
  • RC151 GST/HST credit application for individuals new to Canada
  • Other CRA forms, elections and information returns as your situation requires

Common situations

  • You have just arrived in Canada and need the benefit applications filed before your first return.

  • The corporation paid a dividend and the T5 has to be issued by the end of February.

  • A slip was filed with the wrong figures and needs amending before the CRA raises it.

What I'll need from you

  • The details the form itself calls for — I will send you a short list
  • Prior year filings of the same form, if any
  • Supporting records: payroll totals, dividend resolutions, arrival dates
  • Your business number or SIN, as applicable

Questions

  • I just moved to Canada. Which forms apply to me?

Usually RC151 for the GST/HST credit, and RC66 with the RC66SCH if you have children. Both can be filed before your first tax return, and both are worth filing promptly — the benefits are not retroactive indefinitely.

  • My corporation paid a dividend. Do I file a T5?

Yes. Dividends paid to shareholders are reported on a T5 slip and summary, due by the last day of February following the calendar year in which they were paid.

  • Can you fix a form that was filed incorrectly?

Yes. Amended slips and adjustment requests are ordinary work. It is almost always better to correct a filing than to wait and see whether the CRA notices.
